DATE OF BAN: <Date you got banned, will show in the ban message you get when entering the server>
DURATION OF BAN: <Same as above>

ADMIN BYOND KEY: <The admin who banned you
REASON FOR BAN: <The reason as to why you were banned, also in the ban message shown upon entering the server>
YOUR APPEAL: <This is where you give us a reason as to why you should be unbanned, good tip here is to acknowledge
 that you have read the rules and show us that by giving us which rule you broke and explain it.
 Alternatively explain your situation as to why the ban is wrongly placed or otherwise not justified>
